Patient Representative

The Patient Representative is here to serve as a liaison for you and your family. The patient representative collaborates with all parties involved in a concern, and investigates the situation to help facilitate resolution and respond back to you regarding these issues. The staff member may also help you with questions, ethical issues and any other special needs. Talk with a Charge Nurse about your special needs and if not resolved call a Patient Representative during regular business hours at 324-6000 ext. 67963.

Maternity Registration

Labor and Delivery is located on the 2nd floor. To Pre-Register for a delivery please fill out the form given to you by your physician, and return it to the Emergency Department here at Seton Northwest; or you can call 324-6002, ext. 67482 to pre-register.

Financial Counseling

Seton Northwest offers Financial Counselor assistance for services received at the hospital. Upon confirming eligibility, the Financial Counselor may refer you to various assistance programs/agencies in an effort to assist in meeting your needs. Financial Counselors are also here to assist you with internal assistance programs for those who qualify. For any questions about our Financial Counseling, contact 324-6006.

Patient Responsibility for Payment

Patients and/or their appropriately designated guarantors are ultimately responsible for resolution of the bill for services rendered. Financial counselors are available to discuss charges incurred for your health care while at Seton Northwest should the need arise.

IPP - Initial Payment Program

Every effort is made to determine the source and extent of all possible payment sources, including both third party coverage and the amount for which our patients assume personal responsibility. The Initial Payment Program is a program for patients without insurance who are having outpatient procedures. For eligibility guidelines, contact any Financial Counselor at 324-6006.

Insurance

Seton Northwest Hospital accepts most insurance plans. All deductibles, copays and/or co-insurance's are due at the time of service. Insured patients that present evidence of insurance coverage will have benefits verified and will work closely with the financial counselors to determine the patient's portion of responsibility.

Obtaining your Medical Records

How to obtain your Health Information (medical records) at Seton Northwest Hospital:

1. To receive a copy of your health information, an authorization to release medical information must be signed and dated by the patient or the patient's legal guardian. In addition, proper identification must be presented at time of request.

2. The authorization form is located in the Health Information Management Department located on the 1st floor of Seton Northwest Hospital. The direct number is 324-6493, and records may be obtained 8:00 A.M. to 4:30 P.M., Monday through Friday.

3. Not all records are complete immediately after discharge. Some records are not complete until thirty days after discharge as they are awaiting dictation of history & physical, discharge summary, and operative reports from your physician.

Requestor's may be charged a fee to cover the expense in providing copies of health information. State and federal laws and regulations set these fees effective 9/1/01.

Healthcare Team

During your stay at Seton Northwest Hospital, you will be cared for by a group of highly trained staff dedicated to managing your care. These caregivers may include the following professionals: physicians, registered nurses (RN), licensed practical nurses (LPN), clinical assistants (CA), nurse practitioners, physician assistants, clinical managers, patient representatives, physical therapists, occupational therapists, speech therapists, social workers, case managers, diagnostic staff, pharmacists and dietitians.

Case Management/Social Services

You are assigned a Case Manager and Social Worker when you are admitted to the hospital.. Your Case Manager is responsible for ensuring that each day of your hospitalization is covered by your insurance. If you do not have insurance, your Case Manager and/or Social Worker will work with Seton Northwest's Financial Counselors to assist you in obtaining alternate financial resources. In addition, your Case Manager will work with you, your Physician and your treatment team to ensure that you have an appropriate and safe discharge plan. Your Social Worker will often work with the Case Manager or independently on discharge planning as well. This process begins on the day of your admission. Your Social Worker may also provide supportive counseling to you and coordination of your care and discharge plans with your family, friends or caregivers. If you do not know who your social worker or case manager is and you need to talk to one of them you can ask your nurse to contact them or use your phone to call one of the numbers below:
